An initial consultation will take approximately 2 hours; please plan your time accordingly. During a pediatric appointment, Lindy meets with you and your child for the first 15 to 45 minutes, depending on their age. The remainder of the appointment will be with the parent(s) only. Our waiting area has toys and books for children, but out of concern for safety and security, we cannot allow children to be unsupervised.
Follow-up appointments are schedule for 45 minutes. Children are encouraged to be present unless other accommodations have been recommended.
We request that you arrive 15 minutes early for your first appointment.
For your convenience, forms are provided below in a downloadable .PDF format.
Please print, fill out, and bring with you to your initial consult. We require the New Patient Intake and the HIPAA form.
