We are a consultative practice and function as an out-of-network provider. Therefore, we are unable to bill insurance for your office visit. You will be given documentation necessary for reimbursement through an HSA account or the out-of-network provision in your insurance contract. Payment for the office visit will be due in full at the time of the appointment.
The following information will help you with scheduling and office policies. Please contact our office if you have any questions regarding an appointment. We look forward to meeting you!
Fal River Health Center is open Monday through Thursday from 8:30 to 4:00 and Friday from 8:30 to noon. When the office is closed, we offer confidential voice mail where you may leave messages. Contact information for our twenty-four hour coverage can be found on our phone recording.
The initial consultation will take approximately two hours; please plan your time accordingly.
If you are bringing a child for an initial consultation, please make arrangements for another adult to join you for the appointment. Lindy would like to meet with you and your child together for the first 15 to 30 minutes; the remainder of the appointment will be with the parents. Our waiting area has toys and books for children, but out of concern for our patients' safety and security, we cannot allow children to be unsupervised in the office.
Follow-up appointments are scheduled for forty-five minutes. Children are encouraged to be present unless other accommodations have been recommended.
It is important that you arrive a few minutes early to fill out paperwork. For your convenience some of the necessary forms are provided here in downloadable .pdf format.
Please forward pertinent medical records to the office prior to your initial consultation. These documents often provide valuable information that helps us formulate a treatment plan. Examples of such records include recent lab work, allergy testing, immunization records, and neurologic or educational evaluations.
If you have any questions regarding which documents are relevant, please contact the office.
You and your family are very important to us. As an Integrative Medical practice we are dedicated to providing the highest level of comprehensive healthcare and education. We hope you understand the reason for this cancellation policy.
If you cannot, or choose not to keep a routine appointment, we kindly ask for a 24-hour cancellation notice. A 48-hour notice is requested for an initial consultation.
If a cancellation is not made, you will be charged 50% of the first missed appointment fee. For any subsequent missed appointments, you will be charged the full amount for the time reserved. We do understand that there are extenuating circumstances; a simple phone call is requested to avoid these charges.
Due to the length of time that we dedicate to both initial and follow-up appointments, it can be difficult to fill them without adequate notice. We ask that you please read our cancellation policy carefully. If you have any questions, kindly contact the office.
